About Us:
We are a rapidly growing medical practice launching a Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) program for Medicare and select commercial insurance patients. Our mission is to improve patient outcomes through proactive engagement and technology-driven care.
- Responsibilities:
- Contact new patient referrals via phone/email to introduce RPM/CCM program and answer questions
- Verify insurance eligibility for RPM/CCM coverage
- Obtain and process e-signature consent forms
- Coordinate with device vendor for ordering and shipment (e.g., BP cuff, scale)
- Track device delivery and patient setup status
- Schedule initial and follow-up calls with our nursing team
- Document all interactions in HIPAA-compliant system
- Maintain organized patient onboarding logs and status reports
- Requirements:
- 1+ year experience as a Medical Assistant, Care Coordinator, or similar healthcare role
- HIPAA training and compliance knowledge
- Excellent communication skills (clear, patient-friendly, professional)
- Strong organizational skills and attention to detail
- Reliable internet and ability to work from a private, quiet environment
- Proficient with basic EHR systems, email, and online forms
- Comfortable with patient intake scripts and structured workflows
- Preferred Qualifications:
- Experience with RPM/CCM or telehealth programs
- Familiarity with Medicare and managed care coverage
- Insurance verification experience
