About the position
The RN Consultant position at Liberty Healthcare involves providing technical assistance and training to Service Coordinators conducting assessments using the interRAI-HC tool. This hybrid role requires both fieldwork and remote work, focusing on quality reviews and performance improvement in Home and Community Based Services (HCBS).
Responsibilities
Ā In-person and virtual training and technical assistance with Service Coordinators
,
Ā Applying subject matter expertise with the interRAI-HC tool
,
Ā Ensuring best practices are used in the administration of the interRAI-HC tool
,
Ā Conducting quality reviews of assessments and service plans
,
Ā Proficiency testing
,
Ā Developing and implementing performance improvement plans and monitoring efficacy
,
Ā Collaborating with supervisors, managers, trainers, and educators
,
Ā Data analysis and reporting
Requirements
Ā Registered Nurse (RN) license
,
Ā Experience conducting functional assessments of older adults and adults living with disabilities
,
Ā Experience in Home and Community Based Services (HCBS) or Long Term Services and Supports (LTSS)
,
Ā Experience in case management and service coordination
,
Ā Experience in leadership, management, supervision, or consulting
,
Ā Experience providing technical assistance and training
