About the position

The RN Consultant position at Liberty Healthcare involves providing technical assistance and training to Service Coordinators conducting assessments using the interRAI-HC tool. This hybrid role requires both fieldwork and remote work, focusing on quality reviews and performance improvement in Home and Community Based Services (HCBS).

Responsibilities
Ā• In-person and virtual training and technical assistance with Service Coordinators
,
Ā• Applying subject matter expertise with the interRAI-HC tool
,
Ā• Ensuring best practices are used in the administration of the interRAI-HC tool
,
Ā• Conducting quality reviews of assessments and service plans
,
Ā• Proficiency testing
,
Ā• Developing and implementing performance improvement plans and monitoring efficacy
,
Ā• Collaborating with supervisors, managers, trainers, and educators
,
Ā• Data analysis and reporting

Requirements
Ā• Registered Nurse (RN) license
,
Ā• Experience conducting functional assessments of older adults and adults living with disabilities
,
Ā• Experience in Home and Community Based Services (HCBS) or Long Term Services and Supports (LTSS)
,
Ā• Experience in case management and service coordination
,
Ā• Experience in leadership, management, supervision, or consulting
,
Ā• Experience providing technical assistance and training
