Position Details:
รย Part-time at 3+ hours per day

Key Responsibilities:
รย Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community.
รย Providing care coordination
รย Providing psychoeducation
รย Providing referrals
Requirements:
รย Masterรยs degree in Social Work
รย Active Social Work License
รย Pennsylvania Teaching Certification
รย Virtual experience preferred
