Job Responsibilities:
- Conduct telephonic clinical assessments using approved medical protocols
- Provide recommendations for appropriate levels of clinical care based on clinical judgment and protocols
- Outreach and engage members with programs to promote health and well-being
- Complete clinical eligibility assessments and identify health, condition, or social gaps
Work Schedule:
Monday-Friday: 2 shifts Pacific Time Zone between 8:30 am - 5 pm, then 3 shifts Pacific Time 10:30 am - 7 pm. Quarterly Saturday shifts 8:30 am - 5 pm Pacific time.
Requirements:
- Undergraduate degree or equivalent experience
- Multi-State Compact License
- Outpatient clinical, ICU, ER, PCU, Step-down experience
- Computer proficiency (Microsoft Word, Outlook, and internet)
Preferred Qualifications:
- Call center experience
- Experience with EMR (Electronic Medical Records)
- Recent Case Management experience
- Bilingual - Spanish
