Full-Time | 8-Hour Day Shifts | 13-Week Contract
$2,100 per week for 40 hours
Job Overview:
Northside Hospital Gwinnett is seeking an experienced Care Coordinator (Nursing) for a travel contract in our Gwi-COORD CARE GMCL department. This role involves providing patient-centered care coordination, case management, and utilization review to ensure optimal patient outcomes.
Job Details:
Start Date: 02/24/2025 | End Date: 05/24/2025
Pay Rate: $2,100 per week for 40 hours
Schedule: MondayFriday, 8-hour day shifts
Guaranteed Hours: 40 per week
Overtime: Daily OT after 24 hours | Weekly OT after 40 hours
No Local Candidates (Must be at least 50 miles from Lawrenceville, GA)
Requirements:
? Education: Graduate of an accredited Registered Nursing (RN) program
? License: Current Georgia RN license (GARN)
? Experience: Minimum 2 years in case management, utilization review, or social services
? Skills:
Strong communication skills (written & verbal)
Ability to interact with patients, families, physicians, and hospital staff
Typing proficiency (4045 WPM)
Preferred Qualifications:
? BSN (Bachelor of Science in Nursing)
? 3+ years of experience in case management, social services, or related field
? Certified Case Manager (CCM)
