About the position

Responsibilities


  • Develop and maintain one-on-one relationships with patients.

  • Teach patients how to manage their health issues effectively.

  • Celebrate patient successes in healing and recovery.

  • Conduct patient home visits primarily during weekdays.

  • Complete OASIS documentation with significant support.

  • Work independently while managing patient care.

Requirements


  • Minimum 1 year of acute-care nursing experience.

  • Registered nurse license in good standing.

  • Current CPR certification.

  • Current TB test and Fingerprint Card.

  • Current AZ driver's license and auto insurance.

  • Reliable vehicle for driving to client locations.

  • Ability to work independently.
