- Compensation : $130,000-$190,000 per year (dependent on experience)

- Key Responsibilities :
- Assess patient needs and create personalized home-based physical therapy treatment plans.
- Implement evidence-based interventions to enhance mobility and alleviate pain.
- Monitor patient progress and modify treatment plans for optimal outcomes.
- Educate patients and caregivers on safe exercise practices and mobility strategies.
- Collaborate with a multidisciplinary care team to ensure excellent patient outcomes.
- Maintain timely and accurate documentation using user-friendly digital systems.
- Promote independence and deliver compassionate, supportive care to patients.
- Qualifications :
- Valid and unrestricted California PT license
- At least 1 year of Physical Therapy experience
- Current AHA CPR certification
- Valid Driver's License and reliable transportation required
- Excellent communication, time-management, and clinical reasoning skills
