Peak Health is a fast-growing concierge regenerative medicine company providing in-home and telehealth-based care. We are seeking a detail-oriented, compassionate Case Manager with a background in medical assisting, nursing, or patient coordination to manage patient care and treatment journeys. This is a remote position with flexible hours and opportunity for growth.
- Key Responsibilities:
- Coordinate care and scheduling with patients post-consultation
- Support patients via phone/text with treatment logistics and education
- Ensure proper documentation in EHR and CRM systems
- Track follow-up touchpoints and patient progress
- Liaise with nurse practitioners, sales, and logistics teams
- Preferred Qualifications:
- Background as MA, CNA, LPN, RN, or similar
- Excellent phone and communication skills
- Tech-savvy: comfortable with CRMs, EHRs, telehealth platforms
- Experience in concierge, stem cell, or regenerative medicine a plus
- Bilingual (Spanish) is a plus
Compensation: Competitive pay, performance-based bonuses, and growth opportunities.

Pay: $15.00 - $20.00 per hour
