Perform routine scheduled visits with clients to monitor care and oversee CNAs and caregivers in the home. Maintain detailed and relevant care plans. Assist Clinical and Marketing Teams when your expertise is needed.

Responsibilities
รย Conduct Weekly and Bi-Weekly visits to Clients
รย In person home visits as needed within 2hr time period
รย New clients assessments in a timely manor
รย Maintain accurate and current care plans
รย Coordinate Client needs with their PCP as needed
รย Provide counsel and guidance to family members on managing conditions
Requirements:
รย Active RN License in Virginia or a compact state
รย Driver's License and vehicle
รย Current CPR
